38-3XX 6U7G tube shield? - Rich K. - 07-19-2013

I have a 38-3XX (code 121, whatever that means), and the tube shield for the 6U7G tube (left rear of the tuning capacitor) is missing. It has a base I've never seen before. I tried ordering one from Mark Oppat, but he sent me a square one that doesn't fit that base. Can someone please post a photo of the correct one that I can send him so I can get the right shield?
