Trying to identify model number

Are you going to be putting humty dumpty back together again?? Note the to tubes on the right are reversed the 5Y4 goes on the transformer and the 6F6G should be fitted to the chassis.

Service info can be found here: http://www.nostalgiaair.org/Resources/209/M0013209.htm
It's a code 121.

Here's one that Kelly went a little crazy on, it's a code 122. The difference being the 5Y4 mount to the chassis rather than the top of the power transformer.

I'm going to try or give it a good effort. I'm sure this unit has had previous work done I've been searching for months trying to identify it by pics and schematics so with this new info I've got somewhere to start now.  I haven't touched anything on this unit except wipe the dirt off the tubes I cleaned up the cabinet a bit and added a plexiglass window.
