No Canadian model by that number that I'm aware of.

That's a Model 46-131, I believe you're looking for?
4 tubes: 1LA6, 1LN5, 1LH4, 1A5.

Or is could be a 42-131, also four tubes: 1LA6, 1LN5, 1LH4, 1LA4.
