Walnut veneer.

2. Color of lacquer to use? Highlights?

Medium Walnut, with either Extra Dark Walnut or black lacquer on the edge and bottom trim.

I think it should have the "Ribbed" pattern (#14 from Radio Grille Cloth Headquarters).

4. Any decals?

No, since the PHILCO trademark is on the dial scale.
